Tri-City Health Center Mission Statement

Tri-City Health Center (TCHC) provides a wide spectrum of educational, healthcare, and social services to everyone, without regard to financial position, ethnicity, language, culture, sexual orientation or any other criterion. TCHC is committed to delivering excellent health services in a caring, nurturing, and respectful atmosphere; we strive to promote optimal health, eliminate preventable diseases, and improve the Quality of life for every individual and family in our community. TCHC is a fiscally responsible, independent, not-for-profit community health center and a key member of a vital health service provider network.

HIV Prevention Department Mission Statement

Tri-City Health Center's HIV/HCV Prevention Department is committed to the Education and empowerment of diverse, at-risk populations by increasing the knowledge of HIV/HCV prevention, transmission, testing and treatment through the application of non-judgmental harm reduction principles. Our goal is to increase access to health and human services as well as engage the communities of South, Central and East Alameda Counties to collaborate in appropriately addressing the HIV/HCV epidemics.

TransVision Mission Statement

TransVision's mission is to provide a safe, welcoming, and trustworthy program for transgender women in a supportive and non-judgmental environment. We are committed to providing services that affirm and empower transgender women, as well as providing access to quality health care that includes HIV prevention, testing, and treatment.
With the tools of education and outreach, we challenge the communities of Alameda County to address the stigma and intolerance that is targeted towards transgender women.

What is the TransVision Program?

What Does TransVision do?
TransVision is the only transgender program in Alameda County, and one of the only resources in the county for transgender women.
We are funded by the Office of AIDS.
Other past funding sources have been MAC AIDS Foundation, Kaiser Permanente, Horizons Foundation, and donations from the community. We are a county wide program.

In The Beginning

TransVision was created in November 2002 to improve access to HIV prevention, testing, and care and treatment for transgender women in South/Central and East Alameda County. Prior to the inception of TransVision, there were no HIV services targeting transgender women in the non-urban areas of Alameda County. TransVision was awarded the county contract as the only provider of transgender services by the Office of AIDS Administration in 2005.
TransVision has successfully established contact with women in Alameda County through late night street and venue-based outreach, Internet advertising and networking with organizations in urban areas that are used as resources by transgender women in this area. From 2002 to date, TransVision has had over a 3000 contacts with transgender women through our various program activities.
In 2004, TransVision created a Peer Educator Intern Training Program. This was a very successful first step in the expansion of services for TransVision.

Tri-City Health Center is a community clinic located in Fremont serving the residents of Southern, Eastern, and Central Alameda County. Our accessible services serve high-risk and vulnerable populations by leveraging our unique position as a community clinic to reach into those populations and provide responsive programs that lead to a healthier community.
TransVision is a program in their HIV Services Department, and was created in November 2002 to improve access to HIV prevention, testing, and care and treatment for transgender/transsexual women. The TransVision model is one that recognizes that being transgender/transsexual/gender variant is a gift and that community members deserve the best services possible. Prior to the inception of TransVision, there were no HIV services or any other resource targeting transgender women in the non-urban areas of the County and it is currently the only county funded program for transgender/transsexual women in Alameda.

Tiffany Woods:
Program Coordinator for TransVision, Tri-City Health Center. She created TransVision along with her Program Manager in 2002.

Tiffany Woods is a Health Education Coordinator in the HIV Services at Tri-City Health Center, Fremont, CA. She is currently the Program Coordinator for TransVision, the only transgender program serving transgender women in Alameda County.

She created TransVision along with her Program Manager in 2002. Prior to the inception of TransVision, there were no HIV services targeting transgender women in the non-urban areas of Alameda County.

She is a graduate of the CDC/ASPH Institute for HIV Prevention Leadership (2005) in Atlanta, GA. where she attained scholar of the year. She served as visiting faculty in 2006, 2007 2010 co-teaching Strategic Planning and teaching Special Populations and was a member of the Fellows Advisory Committee (2010).

She is an HIV Testing Counselor and Community Health Outreach Worker (CHOW). She was appointed to the Alameda County/Contra Costa Community Collaborative Planning Council in 2007 and Co-Chairs the Policy, Education, and Review Committee. She is also a trainer for the Harm Reduction Coalition.

She served as the Executive Chair of the Community Advisory Board of the Center of Excellence for Transgender HIV Prevention, UCSF, CA from 2007-2009; and currently is a member of the National Community Advisory Board for the Center of Excellence for Transgender Health, UCSF, CA.

She has been honored with an Excellency in Advocacy Award by Tri-City Health Center in conjunction with Community Health Center Week, 2010, sponsored by the Alameda County Health Consortium and Community Health Center Network.

She has been honored with the Alameda County Prevention and Education Award by the Alameda County Office of AIDS presented at the World AIDS Day Community Services Awards, Dec 1, 2010.

She has done numerous presentations and trainings for health care providers, Planning Councils, college classes, medical students, and agencies on gender, transgender health issues, transgender & HIV, & transgender language and identity. She presented at the US Conference on AIDS in 2008-2009.

She is the producer of Alameda Counties Transgender Day of Remembrance Event held in Oakland, an event held annually on November 20 around the world to remember Trans people murdered through anti-trans violence and hatred. Tiffany is also profiled in the Award Wining Photo-Documentary, Transfigurations, by Santa Cruz photographer Jana Marcus.

She is an active member the Screen Actors Guild. She is Associate Producer in the soon to be released documentary "TransFrancisco", as well being profiled in the film. As an Independent Background Casting Director in Los Angeles, working in independent film, she has cast eight feature films, 2 short films, and participated in varies other projects.

Activities/Services

Some of the activities that TransVison conducts are outreach to transgender women at venues throughout Alameda County, providing health information, safe sex supplies, and referrals. TransVision staff conducts workshops for transgender women on issues such as HIV, safe sex, transitioning, health care, hormones, self-esteem and safety in public spaces. We also provide presentations on transgender issues at colleges and schools, as well as sensitivity trainings to medical providers.
